This policy explains how The Dales Physiotherapy and Rehabilitation Clinic (“we”, “us”, “the clinic”) collects, uses and protects your personal information when you visit dalesphysio.co.uk or use our services.
The Dales Physiotherapy and Rehabilitation Clinic is run by Susan Addison, a Chartered Physiotherapist registered with the Health and Care Professions Council (HCPC) and a member of the Chartered Society of Physiotherapy (CSP). We are the data controller for the personal information described in this policy.

We do not process any card or bank details through this website. Payment for appointments is taken in person or by phone, by cash, card or BACS transfer.

We will never use your health information for marketing, and we do not sell or rent personal information to any third party.
Enquiry and contact form submissions that don’t lead to an appointment are kept for 12 months and then deleted.
Clinical records are kept in line with professional guidance from the Chartered Society of Physiotherapy. For adult patients this is currently eight years from the date of your last treatment. For patients under 18, records are kept until their 25th birthday, or for eight years after their 18th birthday if that is later.
We use a small number of third-party services to run the practice and this website. Where any of these providers process personal data on our behalf, we have chosen providers who meet their own data protection obligations.
We only share your health information with other professionals (for example, a GP or a specialist) where you’ve asked us to, or where we need to for your ongoing care, and only with your consent.
We may also need to disclose information where required by law, for example in response to a court order, or where there is a serious safeguarding concern.
This website may use cookies, small files stored on your device, to help it run properly and to understand how it’s used. Some cookies are essential to the site working. Others, such as those from the embedded Google Map, are set by third parties when that content loads.

Our services are offered to patients of all ages. Where a patient is under 18, we ask for a parent or guardian to be involved in providing consent for treatment and for the personal information collected as part of that care.
